  1. Structured Care & Service Plans – Plan templates define reusable Goals and Steps so staff follow a consistent structure while tailoring each plan to the individual. Actionable steps automatically generate Salesforce Tasks linked to assessments, document uploads, service referrals, and enrollments—turning plans into real, trackable work. 
  1. Plan Versioning & Compliance – “Create new version” clones the current plan into a new record, marks the prior version as inactive, and preserves a full history for audit and regulatory requirements. Plans support electronic signature requests and PDF generation, providing a verifiable consent and records trail.

Configurable Assessment Engine – Admins build assessments in a step-based wizard: pick questions from a shared question bank, organize them into sections with sequencing, define scoring and score translation (e.g., risk tiers, eligibility bands), and configure dependent follow-up questions that appear only when relevant answers are given. 
  1. Smart Scoring & Outcome Tracking – Assessment completion creates Outcome records with total score, responses, and status. Score translation maps numeric ranges to labels or risk levels, driving downstream workflows and eligibility logic. A designated field on the target record (e.g., Risk Level on a Case) can be updated automatically upon completion, keeping all of Salesforce in sync.

Configurable Multi-Object Forms – A dynamic form builder—powered by Apex and LWC—lets admins configure a primary object plus related parent and child objects, select fields and questions, and optionally add signature capture, document upload, and PDF generation. The platform handles create/update ordering across related records with validation and rollback, ensuring data integrity in every submission. 
  1. Shared Question Library & Integrated Workflows – Plans, Assessments, and Forms share the same question bank and Salesforce security model, creating a unified ecosystem. Tasks generated from plan steps point directly to the relevant assessments and document collection forms, so every part of the care journey—from plan goals to assessment outcomes to signed intake forms—flows through a single, connected platform.
